Hydrocele surgeries

Hydrocele of the testicles is a collection of serous fluid around the testicles. The condition is not life-threatening and does not impair the function of the testicles, but it is disturbing if it grows to a large extent. There are several causes of its occurrence, most often injury, inflammation of the epididymis/testicle, testicular carcinoma, the condition after varicocele surgery and hernia.

What about hydrocele?

A hydrocele of the testicles is the accumulation of serous fluid around the testicles. The condition is not life-threatening and does not impair testicular function, but it can become bothersome if it grows significantly. There are several causes, most commonly injury, inflammation of the epididymis/testicle, testicular carcinoma, or a condition following varicocele or hernia surgery. Before surgery, it is essential to perform an ultrasound of the scrotum to rule out carcinoma, hernia, and other pathologies that may clinically present as a hydrocele. We distinguish between non-communicating and communicating hydrocele, as well as funiculocele.

How is the surgery performed?

Hydrocele surgery is performed under general or spinal anesthesia. The surgeon makes an incision above the scrotum and isolates the hydrocele. The sac is then opened, the excess membrane around the testicles is removed, and the area is sutured. A drain is inserted, and the scrotum is closed using absorbable stitches.

Why is surgery necessary?

Hydrocele surgery is not an urgent procedure, as it does not affect testicular function and is not life-threatening. It is recommended when the patient experiences pain, a feeling of tension in the scrotum, or dissatisfaction with its cosmetic appearance.

Possible Complications of Hydrocele Surgery

Internal medicine complications
Although it is a small, minimally invasive surgical procedure that usually does not last longer than 60 minutes, the procedure can cause internal medical complications such as DEEP VEIN THROMBOSIS, PULMONARY EMBOLISM and ACUTE MYOCARDIAL INFARCTION. The risk of developing deep vein thrombosis with or without pulmonary embolism is increased in elderly patients with arrhythmias - atrial fibrillation and in people with blood clotting disorders. The risk is also increased in patients who are bedridden for a long time due to pain. As a rule, you will receive medication to prevent the above-mentioned events. The best protection is early postoperative getting up and walking. If such a complication should occur during treatment, our professional staff is trained to recognize and treat it correctly, and in case of the need for invasive diagnostics or therapy, we will immediately transfer you to the nearest adequately equipped internal medical center.
Surgical Complications
  • Bleeding: During and after hydrocele surgery, bleeding into the scrotum may occur. To prevent the formation of blood clots, the surgeon places a drain in the scrotum during the procedure. Blood clots usually dissolve spontaneously within a month. In cases of infection or a larger hematoma, surgical removal of the clots may be necessary.

  • Infection: In surgeries involving the genital area, it is not always possible to ensure complete sterility. As a result, inflammation of the scrotal tissues may occur. How do I prepare for the procedure?

On the day of the procedure, you must not consume any food or fluids.

A few days before the surgery, you need to stop taking medications that increase the risk of bleeding, such as: MARIVARIN, MAREVAN, SINTROM, apixaban (ELIQUIS), dabigatran (PRADAXA), or rivaroxaban (XARELTO). Before the procedure, a preoperative assessment at the antithrombotic clinic is MANDATORY.

Bring a written INR report showing a value of 1.3 or lower, issued no more than one or two days before the procedure. Patients taking PLAVIX can discontinue therapy only with the approval of a cardiologist or neurologist. If discontinuation is not possible, we will try to adjust the surgery date. Aspirin 100 Protect does not necessarily need to be stopped before the procedure, but it is recommended to avoid taking it for at least seven days prior to surgery.

Be sure to bring a list of all medications you are taking with you.

Arrange transportation home, as you will not be able to drive yourself and excessive physical activity would increase the risk of postoperative bleeding. We advise against heavy physical activity for 4-6 weeks after surgery.

After the procedure

After the surgery, you will be transferred to the recovery room, where you will remain under observation until the effects of the chosen anesthesia wear off. You will receive pain relief medication. In most cases, you will stay in the hospital overnight and be discharged the following day. Pain is usually mild and typically subsides completely within 10 to 14 days. Upon discharge, you will be prescribed medication to help manage any discomfort.

We advise

Drink plenty of fluids. For a few weeks after the surgery, eat cooked foods rich in fiber to avoid constipation and straining. In case of constipation, you may use Donat Mg or suppositories. You can resume blood-thinning medications 2–4 weeks after surgery if there are no signs of bleeding in the scrotum. Avoid heavy physical exertion, driving, and sexual activity for at least 4–6 weeks after the procedure.

Results

Hydrocele surgery usually relieves symptoms immediately. After the procedure, swelling of the scrotum can be expected to persist for at least 4 weeks.
