Autonomic nervous system testing

The autonomic nervous system regulates numerous vital functions that take place automatically, without our conscious control. These include the regulation of blood pressure, heart rate, breathing, sweating, and the functioning of the digestive and urinary systems. Autonomic nervous system testing is a non-invasive examination that assesses the function of the cardiovascular part of the autonomic nervous system; its two branches – the sympathetic and the parasympathetic system – can be assessed separately. During the examination we monitor blood pressure, heart rate and breathing at rest and during standardised challenges such as deep breathing, muscle activation and standing.
The examination is helpful in people with transient loss of consciousness (syncope), dizziness on standing, suspected postural orthostatic tachycardia syndrome (POTS), orthostatic hypotension, and autonomic nervous system disorders in diabetes, Parkinson’s disease, polyneuropathies or other neurological diseases.

About the examination

The examination is safe, painless and carried out under the constant supervision of medical staff. During the examination the patient lies on a table, with electrodes placed on the chest to monitor the ECG and a sensor on the finger for continuous blood pressure measurement. Several standardised tests of autonomic nervous system function are performed and individually adapted to the patient (active standing test, deep breathing test, handgrip test, diving reflex and others). The whole examination usually takes 45–60 minutes.

A visit is recommended for symptoms such as frequent headaches, dizziness, loss of sensation, weakness in the limbs, visual disturbances, problems with balance or memory. A neurologist can also help with suspected diseases such as multiple sclerosis, Parkinson's disease or epilepsy.

The doctor will first discuss your problems and medical history. This will be followed by a clinical neurological examination, and if necessary, additional tests such as EMG, MRI or EEG will be ordered. Based on the findings, a plan for further treatment or management will be developed.
