Laser enucleation of the prostate (ThuLEP) – a modern solution for the reduction of benign prostatic enlargement

Laser enucleation of the prostate (ThuLEP) is an innovative, minimally invasive method for the treatment of benign prostatic hyperplasia (BPH), i.e., non-cancerous prostate enlargement. At the Center for Modern Urology, we perform this method in cooperation with the most advanced surgical medical center in Slovenia – MD Medicina. The surgeries are performed by Sebastian Jeršinovič, M.D., who acquired his endourological surgical expertise at the renowned University Clinic in Tübingen, Germany.

What is ThuLEP (Thulium Laser Enucleation of the Prostate)?

To reduce the prostate, we use a laser beam that precisely peels the prostate tissue away from its capsule (so-called enucleation). The excess prostate tissue is then removed from the bladder using a special instrument (morcellation). This method is considered an advancement over the classic TURP (transurethral resection of the prostate), as it allows more thorough tissue removal, long-term results, lower risk of bleeding, and faster recovery. With this method, we are not limited by prostate size, as even prostates larger than 200 g can be removed.

Advantages of the ThuLEP method

High effectiveness: precise removal of prostate tissue, leading to an improvement in lower urinary tract symptoms such as difficulty urinating, weak stream, and nighttime urination.

Low risk of bleeding: laser technology provides excellent control over bleeding, which is especially important for older patients and those with blood clotting disorders.

Faster recovery: shorter hospital stay and faster recovery compared to traditional methods such as TURP. Most patients are discharged home on the first day after the procedure.

Long-lasting effect: long-term studies confirm lasting symptom relief and improved quality of life for patients.

Who is ThuLEP suitable for?

It is suitable for men with:

  • lower urinary tract symptoms due to enlarged prostate (> 60 g)
  • inefficacy and side effects of medical therapy
  • desire for a permanent solution without lifelong medication

What are the possible complications after surgery?

ThuLEP is a modern, minimally invasive, and safe method for treating benign prostatic hyperplasia, with a high rate of long-term success and a low risk of serious complications. Despite its high effectiveness and low risk profile, certain complications may occur. These are rare, mostly mild and temporary, but it is important to be aware of them.

What are urinary stones?

Urinary stones form due to the crystallization of minerals in the urine. Smaller stones (under 5 mm) often pass spontaneously, while larger ones may cause:

Treatment with the thulium laser

Thulium laser allows precise and safe fragmentation of stones (lithotripsy) in the kidney, ureter, and bladder. Compared to traditional methods, it allows highly controlled fragmentation even in very delicate parts of the urinary tract.

Dr. Sebastian Jeršinovič is a urology specialist, trained in modern minimally invasive and robot-assisted surgical procedures. Comparison with other conventional treatment methods

Laser endoscopic lithotripsy offers numerous advantages over conventional methods such as extracorporeal shock wave lithotripsy (ESWL) or waiting for the spontaneous passage of the stone combined with medical therapy. These methods are unsuccessful in the vast majority of cases and can be very painful.

Procedure of laser lithotripsy

With a special high-resolution (UHD) camera, the stone in the kidney, ureter, or bladder is identified and, under direct vision, treated with a laser fiber. Dust particles are rinsed out, while larger stone fragments are physically removed with a special basket. This allows us to send a stone sample for analysis, based on which preventive treatment can be indicated during further nephrological management to prevent the recurrence of kidney stones. The procedure is performed under general anesthesia, in most cases on an outpatient basis, which means patients can be discharged home the same day. During the procedure, a ureteral stent (JJ stent) is placed for a few days and later removed painlessly on an outpatient basis. The patient is usually fit for work by the second or third day after surgery.

Complications after laser stone removal are rare but possible:
- residual stone requiring repeat surgical procedure
- urinary tract infection treated with antibiotics
- ureteral injury that may lead to stricture and require additional treatment
