General anesthesia

General anesthesia is anesthesia in which the patient sleeps, is unaware of what is happening around them, and is free from pain. The patient remembers nothing. It is a precisely controlled and monitored state of unconsciousness.

The patient is put to sleep either by administering medication into a vein (a sedative, analgesic, and, if necessary, a muscle relaxant) or by inhaling a mixture of oxygen, air, and volatile anesthetic through a face mask. In both methods, the patient falls asleep within a few minutes, and anesthesia can then be maintained for as long as the operation lasts, either by continuously delivering the anesthetic into a vein via a special pump or by supplying volatile anesthetic through the airways. For certain operations, the surgeon requires muscle relaxation to facilitate the procedure, so during general anesthesia, the anesthesiologist administers drugs (muscle relaxants) to suppress reflexes and cause muscle relaxation.

During deep unconsciousness following the induction of anesthesia, the tongue becomes relaxed and can obstruct the airway, and due to the relaxation of the respiratory muscles, the patient stops breathing. Therefore, the anesthesiologist must secure the airway by inserting a plastic tube (endotracheal tube) into the trachea or a laryngeal mask into the throat area, thereby temporarily replacing the patient’s breathing with artificial ventilation using the anesthesia machine. This delivers a mixture of oxygen and air, and in the case of inhalation anesthesia, also a volatile anesthetic. The anesthesiologist inserts the tube or laryngeal mask after the patient is already asleep and removes it at the end of the operation or anesthesia, when the patient begins to breathe again on their own and reflexes return.

The patient is usually unaware of this, but may temporarily be slightly hoarse or feel mild throat discomfort. Awakening from general anesthesia is rapid (immediately after the end of the surgical procedure) and, thanks to the use of modern anesthetic agents, pleasant. The anesthesiologist ensures that the patient experiences no pain after the surgery is completed.
